The Quick Answer
A South African leave application form needs six things: the employee's details, the leave type (matching the BCEA categories), the exact dates and working-day count, the supporting proof where the law requires it, a balance check against the leave record, and a signed decision. The balance check is the part almost every free form skips - and it is the part that prevents the payout disputes.

Copy the full form text, paste it into your letterhead in Word or Google Docs, and print. Every field below is in it.

A leave request that is checked against the balance before it is approved is the difference between an approval and a payout dispute later.
What Each Field Does
Leave type, matched to the BCEA
The form lists the statutory categories by section - annual (s20), sick (s22), family responsibility (s27) and the post-Van Wyk shared parental pool - so the request is classified correctly on day one, not reclassified in a dispute.
Dates and working-day count
First day, last day, return date and the number of working days. The working-day count is what hits the balance and payroll; writing it down kills the “I thought the Friday didn’t count” argument.
The proof checkbox
Sick leave beyond two consecutive days needs a medical certificate (BCEA s23); family responsibility leave allows the employer to ask for reasonable proof. A checkbox at request time beats an email chase three weeks later.
The balance check
Balance before, days requested, balance after - and where the balance came from. This is the field that connects the request to your leave register, and the one that makes a negative balance visible before it is approved.
The decision, with a reason if declined
Annual leave timing may be refused on genuine operational grounds - but the reason must be real and recorded. An undocumented refusal pattern is how employers end up explaining themselves at the CCMA.
The BCEA Rules Behind the Form
| Leave type | Entitlement (5-day week) | Proof the employer may require |
|---|---|---|
| Annual | 15 working days per cycle (accrues 1.25/month) | None - timing by agreement |
| Sick | 30 days per 36-month cycle | Medical certificate after 2+ consecutive days |
| Family responsibility | 3 days per year, no carry-over | Reasonable proof of the event |
| Parental (shared pool) | 4 months + 10 days shared between parents | Written election of the split; birth/medical certificate |

A Clean Approval Process in 4 Steps
Employee submits the form before the leave
Set the notice standard in your policy (twice the leave length is a common rule) and apply it consistently. Same-day sick leave is the exception - the form follows the absence.
Approver checks the balance before deciding
Against the leave register, not memory. If the balance would go negative, that is a conversation before approval, not a deduction argument after.
Decision recorded, both parties sign
Approved or declined with a reason. A photo of the signed form on the employee record is enough - the point is that the record exists.
Tracker and payroll updated the same day
The approved days go into the taken column and to payroll before the next run. Every leave dispute audit trail has a gap exactly here.
5 Mistakes That Cause Disputes
WhatsApp approvals
A thumbs-up emoji is an approval you cannot produce in a dispute. If leave is approved in chat, the form still gets completed - the chat is the request, the form is the record.
Approving without the balance
Negative balances are discovered at resignation, when the over-taken days become a deduction argument the BCEA heavily restricts.
Treating parental leave as maternity leave
Since October 2025 there is one shared pool. A form (or policy) that only offers “maternity” misclassifies every father’s and adoptive parent’s request.
No return date
Open-ended leave is how “on leave” quietly becomes abscondment - or the employer’s word against the employee’s about when work was due to resume.
The form stays in a drawer
If payroll and the tracker aren’t updated, the form protected nobody. The update is part of the approval, not an admin task for later.
Or Approve Leave in 5 Seconds, With the Balance Built In
This form exists because paper needs a process. In Synthro, the process is the system: the employee requests leave from their phone, the approver sees the live BCEA balance next to the request, one tap approves it, the tracker updates itself, the team calendar and Google Calendar sync, and payroll gets the deduction automatically. The audit trail the CCMA asks for is generated by using the product, not by filing.
Frequently Asked Questions
Can I download this leave application form as a Word document?
Yes. Enter your name and email above and we send the form as an editable Microsoft Word (.docx) document plus a print-ready PDF. It opens in Word, Google Docs and LibreOffice - drop it onto your letterhead and it is ready to use the same day.
Can an employer refuse a leave application in South Africa?
For annual leave, yes - timing is agreed between employer and employee, and failing agreement the employer may determine it, provided leave is granted within six months of the cycle ending and the refusal is based on real operational requirements. Sick leave and family responsibility leave cannot be refused when the entitlement and proof requirements are met.
When is a medical certificate required for sick leave?
When the employee is away for more than two consecutive days, or has been absent on more than two occasions in an eight-week period (BCEA Section 23). The certificate must be issued and signed by a practitioner registered with a professional council. Without it, the employer may treat those days as unpaid.
How much notice must an employee give for annual leave?
The BCEA does not fix a notice period for annual leave applications - it is governed by your leave policy or contract. A common, defensible standard is notice of at least twice the length of the leave requested (e.g. two weeks’ notice for one week of leave), applied consistently.
What counts as family responsibility leave?
Three paid days per year (for employees working at least four days a week for the same employer for more than four months) when the employee’s child is sick, or on the death of a spouse or life partner, parent, adoptive parent, grandparent, child, adopted child, grandchild or sibling. The employer may require reasonable proof. It does not accumulate or carry over.
