What Each Field Does
Leave type, matched to the BCEA
The form lists the statutory categories by section - annual (s20), sick (s22), family responsibility (s27) and the post-Van Wyk shared parental pool - so the request is classified correctly on day one, not reclassified in a dispute.
Dates and working-day count
First day, last day, return date and the number of working days. The working-day count is what hits the balance and payroll; writing it down kills the “I thought the Friday didn’t count” argument.
The proof checkbox
Sick leave beyond two consecutive days needs a medical certificate (BCEA s23); family responsibility leave allows the employer to ask for reasonable proof. A checkbox at request time beats an email chase three weeks later.
The balance check
Balance before, days requested, balance after - and where the balance came from. This is the field that connects the request to your leave register, and the one that makes a negative balance visible before it is approved.
The decision, with a reason if declined
Annual leave timing may be refused on genuine operational grounds - but the reason must be real and recorded. An undocumented refusal pattern is how employers end up explaining themselves at the CCMA.

Refusals, Shutdowns and the Situations the Form Must Survive
A leave form earns its keep in the awkward cases, not the easy ones. These are the four that generate most of the disputes:
Refusing an annual leave request
Timing is agreed between the parties; failing agreement, the employer may determine when leave is taken — but statutory leave must still be granted within six months of the cycle ending, and a refusal needs a real operational reason recorded on the form. "Declined" with no reason is what a commissioner reads as arbitrary.
The December shutdown
An employer may schedule annual leave to coincide with a shutdown if the contract or policy provides for it — that is the employer determining timing. The catch is arithmetic: an employee without enough accrued days cannot be forced into a negative balance. The options are agreed unpaid leave or leave granted in advance with a written deduction agreement, and the form should record which one was chosen.
Sick during annual leave
Annual leave may not run concurrently with sick leave. An employee who falls ill during annual leave and produces a valid medical certificate has those days converted to sick leave, and the annual days go back on the balance — a rule that only works if the form and the tracker are updated, not just the group chat.
Leave during a notice period
The BCEA prohibits annual leave during notice, in both directions: the employer cannot set notice off against leave to avoid the payout, and a resignation letter cannot declare remaining leave to be the notice. Employee submits the form before the leave
Set the notice standard in your policy (twice the leave length is a common rule) and apply it consistently. Same-day sick leave is the exception - the form follows the absence.
Approver checks the balance before deciding
Against the leave register, not memory. If the balance would go negative, that is a conversation before approval, not a deduction argument after.
Decision recorded, both parties sign
Approved or declined with a reason. A photo of the signed form on the employee record is enough - the point is that the record exists.
Tracker and payroll updated the same day

5 Mistakes That Cause Disputes
WhatsApp approvals
A thumbs-up emoji is an approval you cannot produce in a dispute. If leave is approved in chat, the form still gets completed - the chat is the request, the form is the record.
Approving without the balance
Negative balances are discovered at resignation, when the over-taken days become a deduction argument the BCEA heavily restricts.
Treating parental leave as maternity leave
Since October 2025 there is one shared pool. A form (or policy) that only offers “maternity” misclassifies every father’s and adoptive parent’s request.
No return date
Open-ended leave is how “on leave” quietly becomes abscondment - or the employer’s word against the employee’s about when work was due to resume.
The form stays in a drawer
If payroll and the tracker aren’t updated, the form protected nobody. The update is part of the approval, not an admin task for later.

Shift and part-time workers
The BCEA accrual also runs in hours — one hour of leave per 17 hours worked — which is the honest way to track staff whose days vary. Add an "hours" line to Part B and calculate the balance in the same unit the roster uses; converting between days and hours at approval time is where balances silently corrupt.
The 24-hour rule, not the job title
Every employee working more than 24 hours a month gets BCEA leave — casual, part-time, fixed-term or permanent, whatever the contract calls them. If your form only exists for "permanent staff", the casual who worked every Saturday for two years still accrued leave; the form just never recorded it.
Keep the forms for at least three years
The BCEA requires employment records, leave records included, to be kept for three years — and a leave dispute referred months after an exit is decided on exactly these forms. A photographed copy on the employee record satisfies this; a WhatsApp thread does not.
POPIA and the medical certificate
A medical certificate is health information — special personal information under POPIA. Store it with the leave form on the employee record with restricted access, and never circulate the diagnosis with the approval ("sick leave approved" is what the team needs to know; the condition is not).

Can an employer refuse a leave application in South Africa?

What counts as family responsibility leave?
Three paid days per year (for employees working at least four days a week for the same employer for more than four months) when the employee’s child is sick, or on the death of a spouse or life partner, parent, adoptive parent, grandparent, child, adopted child, grandchild or sibling. The employer may require reasonable proof. It does not accumulate or carry over.
